Request a Performance Document

All MaPS and most Support Staff (operating-funded, salaried employees who do not have a defined end-date for their appointment and work 0.5FTE or greater) automatically receive an online Annual Performance Review Document. Annual Performance Review Documents for employees who do not meet the criteria can be created upon the Manager's request.

If you are submitting the request for multiple employees, you can submit one form for all employees with their information separated with commas. Please note, sending multiple forms will result in the number of corresponding requests opened in the system and increased volume may affect response times.

Please note: 2026-27 annual review performance documents are anticipated to be available early July 2026.

Provide details including the reason for the request, the employee's staff type (MaPS or AUPE), the type of document (probation or annual), the document performance year, and any other relevant information.
