Uni Dollar/Food Funds Refund Request Form
This is a request to have Unicard balance refunded
Important:
Requests submitted between April 15 and 30 will not be processed until after May 1.
You will be notified via email, with next steps, 10 business days after the form is received.
Refund requests of $20 or less will not be refunded. You are encouraged to spend the funds at a location on campus or donate the funds to the Campus Food Bank.
