I hereby consent to participate as a subject in a research project entitled "Affective and academic results of computer programming experience" conducted by John Mueller and Michele Jacobsen of the Department of Educational Psychology at the University of Calgary. I can contact the project director at 403-220-5664 or by e-mail (mueller@acs.ucalgary.ca).
NOTE: participation is open to anyone, regardless of computer expertise, age, etc.
I understand that I will complete a questionnaire having to do with computer programming courses that I have taken, and questionnaires concerning my reactions to taking tests and my self confidence.
I understand that my participation is completely voluntary, and that I am free to withdraw from the study at any time I choose, without penalty.
I understand that this project is not expected to involve risks of harm any greater than those ordinarily encountered in daily life. I also understand that it is not possible to identify all potential risks in any procedure, but that all reasonable safeguards have been taken to minimize the potential risks.
I understand that the results of this project will be coded in such a way that my identity will not be physically attached to the final data that I produce.
I understand that the results of this research may be published or reported to government agencies, funding agencies, or scientific groups, but that my name will not be associated in any way with any published results.
OK, with the legalese out of the way, the next part of the project requires you to respond to some questions by selecting one of various alternatives. You won't need a pencil, but with your browser you will click on the window to pull up a pop-up menu of choices and make the selection that way.
The display will be ugly with something like LYNX, but it should work; just move the down-arrow key, press Return, and the popup menu should appear.
